Any child who will be attending one of our events will need to have the General Liability Waivers completed by a parent/caregiver before they can join in on the fun. This only needs to be done one time to enjoy all of our events. You will receive an email confirmation after submitting.

By allowing yourself and/or a child in your care to participate in activities hosted by, sponsored by, and or taking place at Florida Keys Healthy Start Coalition (hereinafter “COALITION”), the parent or guardian, or authorized representative thereof (hereinafter “PARENT”) agrees to the following terms, general waiver of liability, assumption of risk, and indemnification.
